Please complete your intake and consent forms including COVID - 19 Pre Visit Questionnaire online prior to your appointment (the forms look the same in the email but once opened are different) and arrive for your scheduled time EARLY, as we cannot guarantee treatments for late arrivals. If you would like additional treatments to what you have been booked in for, please inform our staff ahead of your appointment.
If you are driving, we suggest extra time as there is no longer street parking in front of the clinic, as well as construction in the area. There is some pay parking on Spruce Street, at the Toys'R'Us, in the alley behind, or in the alley across from the clinic.
**We require at least 48 hours notice for any cancellations or changes to your appointment or for NO SHOW a $50 fee will be incurred**
